参赛表格可从校院宣传或http://5thivsc.blogspot.com取得( 可自行复印 )。
Application forms can be obtained during on-campus promotion, or to be downloaded from our website at http://5thivsc.blogspot.com. Application forms may be photocopied.
每份作品只能填写在一份表格上。
Each entry can only be submitted once.
报名费为RM20，请用汇票 (wang pos, bank draft and cheque) 并注明"TEH SHEEN JEFF, NG YI WEI, SAW JIAN PEY"，或直接输入 "TEH SHEEN JEFF, NG YI WEI, SAW JIAN PEY" CIMB用户。
Registration fee is RM20 per entry, please use a bank draft (wang pos, bank draft and cheque), made payable to "TEH SHEEN JEFF, NG YI WEI, SAW JIAN PEY" or you may also bank in directly to our CIMB account at "TEH SHEEN JEFF, NG YI WEI, SAW JIAN PEY"

1.每份报名表格必须附上大专校园的印章。
All entries must be accompanied with the signature of each university or college’s authorised person and campus’s stamp.
2. 每一首参赛作品需录制成光碟DEMO (必须附加乐器伴奏), 并附上歌词6份。
Every submission must be accompanied with a DEMO (must accompanied with instrument), and 6 sheets of lyrics.
3. 截止日期 ：2010年3月1日
Due date : 1st March 2010
4. 所有参赛作品须连同支付报名费的参考号码于截至日期前寄至：
